Health Services
ACCOMMODATIONS FOR MEDICAL CONDITIONS & INJURIES:
Any students requiring specific accommodations in the school setting for medical conditions and/or injuries require a recent doctor’s note (within the last year) on file in the Health Office indicating the doctor’s recommendations. Doctor's notes must be updated each school year.
PE EXEMPTIONS:
PE excusals must be written by a physician and submitted to the Health Office and PE teacher.
ABSENCES FROM SCHOOL:
A doctor’s note is required in order to excuse any absences (including early dismissals). The doctor's note must state the specific date(s) of absences in order to be medically excused. Absences without a doctor's note cannot be medically excused.
MEDICATION REMINDER:
Students are NOT allowed to carry ANY medication (this includes any over-the-counter medication such cough drops, Tylenol, and Motrin). Exceptions are emergency medications (inhalers, Epipen, or Glucagon).
If your student needs medication administered during the school day (including cough drops, Tylenol, or Motrin), medication must be kept in the Health Office and paperwork must be signed by the parent/guardian AND A DOCTOR. Paperwork needs to be renewed EVERY SCHOOL YEAR. Forms can be found on the right under Health Services Forms.
2025-2026 School Year
We are not accepting physical documents in the Health Office. ALL DOCUMENTS MUST BE UPLOADED INTO THE POWERSCHOOL PORTAL WHEN REGISTERING.
FRESHMEN
-Recent Illinois physical (from this year)
-Recent Dental Exam (from this year)
All documents must be completed when uploading into the portal – appointment cards will not be accepted in place of documentation.
Students missing either health document listed above will not be able to receive a device or begin classes in August.
SENIORS
-Proof of the Meningitis vaccination received after age 16
Students missing proof the vaccination will not be able to begin classes in August.
TRANSFER STUDENTS
From Illinois school:
-Illinois physical and vaccination record
-Current dental exam (9th grade ONLY)
From Out of State or Out of Country:
-Illinois physical and vaccination record
-Eye exam
-Current dental exam (9th grade ONLY)
Students missing any health document listed above will not be able to receive a device or begin classes in August.
