Registration

All required medical forms must be uploaded into the PowerSchool portal. WE ARE NOT ACCEPTING PAPER COPIES DROPPED OFF OR FAXED TO THE SCHOOL.

 

All required medical documentation must be uploaded before registration can be completed - we cannot accept appointments in place of medical documentation. If a student is missing required health documentation, he/she will be unable to begin classes in August until the documentation is uploaded.

 

All incoming FRESHMEN are required to upload a recent physical (within the last year) on the correct form - the Illinois Certificate of Child Health Examination (NOT the Sports Physical form) - and a recent dental form (within the last year).

 

All incoming SENIORS are required to upload proof of the Meningitis vaccination received AFTER age 16.

All students entering Argo from OUT OF STATE/COUNTRY are required to upload a recent physical (within the last year) on the correct form - the Illinois Certificate of Child Health Examination - and a recent eye exam (from the last year).

 

Students cannot be cleared to begin classes without the required forms completed.
