Students are NOT allowed to carry ANY medication (this includes any over-the-counter medication such cough drops, Tylenol, and Motrin). Exceptions are emergency medications (inhalers, Epipen, or Glucacon).
If your student needs medication administered during the school day (including cough drops, Tylenol, or Motrin), medication must be kept in the Health Office and paperwork must be signed by the parent/guardian AND A DOCTOR. Paperwork needs to be renewed EVERY SCHOOL YEAR. Forms can be found on the right under Health Services Forms.
2022-2023 School Year
All freshmen will be required to turn in a recent physical (within the last year) BY JUNE 30th. If paperwork is not received by June 30th, your student will not be able to receive a device or begin classes in August. Freshmen are required to turn in a recent dental (within the last year).
All seniors will be required to turn in proof of a Meningitis vaccination after age 16, BY JUNE 30th. If paperwork is not received by June 30th, your student will not be able to receive a device or begin classes in August.
From Illinois school - bring in 9th grade physical AND copy of immunization record
From out of state or out of country - Illinois physical AND eye exam
We CANNOT accept doctor's appointments in place of documentation. All documentation MUST be received by June 30 to prevent exclusion.
